Who needs this insurance?
Spanish law requires comprehensive private health insurance for any long-stay visa or residency permit. These are the most common profiles:
Digital nomad
You work remotely for companies or clients based outside Spain and want to live here legally. The UGE requires insurance with no co-pays, no waiting periods and a certificate compliant with Spain’s Startup Act.
Non-lucrative residency
You don’t work in Spain and have sufficient financial means. The consulate or immigration office requires comprehensive private health insurance as a mandatory condition.
Family reunification
You’re joining a family member who is already a resident in Spain. Each person applying for the authorisation needs their own accredited medical coverage.
International student
You’re studying in Spain for more than 90 days. Health insurance is compulsory for a student visa.
Other visas
Researchers, entrepreneurs, investors and any other profile applying for a residency permit that requires comprehensive private health insurance.
The 5 requirements set by Spanish authorities
Not just any insurance will do. Spanish authorities are strict about the type of coverage they accept. Your policy must meet all of the following:

No co-payments
You cannot be required to pay any additional amount per consultation, test or procedure. Coverage must be total. A policy with co-pays is grounds for rejection.

No waiting periods
Coverage must be active and fully effective from the first day, including specialists, hospitalisation and surgery.

Full coverage across Spain
The insurance must guarantee access to care in every province and region, equivalent to Spain’s public health system.

Repatriation included
Many visas — especially the digital nomad visa — explicitly require repatriation coverage to your home country.

Authorised insurer in Spain
The company must be registered with Spain’s Dirección General de Seguros. Generic travel insurance or international policies are not accepted.
Travel insurance, policies with high co-pays and partial coverage are systematically rejected by the UGE and consulates. This is one of the most common reasons for visa denials.

No. This insurance is taken out with your passport. A NIE is not required at the time of contracting.
Yes. The certificate explicitly states that the policy has no co-pays, no waiting periods, no deductibles and provides full coverage in Spain. It is valid both for applications made in Spain through the UGE and through your country’s Spanish consulate.
The first year must be paid annually. Instalment payments are available from renewal onwards, or from the start if you have a Spanish bank account.
You can pay the annual premium by bank transfer or card. A Spanish account is not required to take out the policy.
The certificate is issued as soon as the policy is formalised. There is no waiting time.
Yes. A spouse and children can be added as additional insured members. Each person can receive their own individual certificate if required.
Yes. It includes emergency coverage abroad for trips of up to 90 consecutive days.
The policy renews automatically each year. You will be able to obtain a new certificate at each renewal to present to immigration authorities.
